Release of Liability

Participant under the age of 18

By signing this waiver, I grant permission for the child named above to participate in and engage in the NLA

Swish basketball activities including but not limited to camps, clinics, evaluations, tryouts, practice, and

tournaments. My child is physically fit and mentally able to participate in these activities. I acknowledge that

there are certain risks involved in said activities and have discussed them with my child if necessary.

 

I discharge, waive, and release the NLA Swish organization, its affiliates, sponsors, coaches, and volunteers of all

responsibilities for any injuries, to body or property, which may occur to my child during participation or travel

to these activities.

 

I also release all camp, clinic, tryout, evaluation, practice, and game sites, including its employees and agents,

person transporting my child to or from camp, clinic, tryout, evaluation, practice, game or activities, from all

liability or claims in case of accident, injury or death. I carry personal medical insurance for my child in case of

accident, injury or illness. In the event of an emergency in which I or the alternate contact, cannot be reached, I

authorize the adult leaders to make medical decisions for my child, and to administer first aid if deemed

necessary.

 

I represent that I am the parent/guardian of __________________________________, who is under 18 years of

age. I have read the Permissions & Waiver/Release of Liability Form and I am fully aware of it contents. I give

permission for the child named above to participate fully in the activities of NLA Swish basketball team.
